Welcome to the CCPS Employees Wellness Program: Pathways to Enhanced Health
Pathways to Enhanced Health is the District's wellness program, which is administered by Community Health Partners (CHP). This is a voluntary program focused on promoting CCPS employees’ health and well-being through preventive care opportunities, i.e., incentive activities.
Incentive activities include bloodwork, biometric screening and Wellness Consult(s), if indicated, and age/gender-based preventive screening. Those who complete incentive activities may qualify for lower deductibles and out-of-pocket expenses. Our goal is to support our employees in reducing their health care cost by providing the tools, information and support to assist them in living a healthier lifestyle. All incentive activities are designed to promote good health and minimize or manage chronic or serious medical conditions.
