SmartChoice is a free benefit available to employees of Collier County Public Schools (CCPS) or Collier County Sheriff’s Office (CCSO) who are covered by their insurance plan. It is a supervised health and wellness education program designed for anyone who has or is at high risk to develop a chronic health condition such as high blood pressure, diabetes, heart disease, hyperlipidemia (high fats in the blood) and/or obesity.
SmartChoice offers healthy lifestyle programs individualized to the employee based on his/her self-identified opportunities. The SmartChoice non-bariatric programs consist of six sessions over a 6-12 month timeframe and the bariatric program includes monthly sessions over 12 months. Each program is championed by a licensed healthcare provider who is a resource expert on chronic health conditions, healthy eating/nutrition, exercise and/or stress management. All sessions are one-on-one and support the meaningful and realistic goals created by the employee during the first session.
SmartChoice is the right choice in pursuing a healthier lifestyle.
