Community Health Partners is a Preferred Provider Network serving customers in Collier and southern Lee counties. Our local comprehensive provider network is comprised of board certified multi-specialty physicians, local hospital systems and ancillary healthcare providers and facilities. Community Health Partners contracts with local employer groups, insurance companies and other networks to provide employees and their beneficiaries with access to quality medical services and modest out-of-pocket expenses.
Community Health Partners also delivers medical services in the areas of utilization management, case management, disease management and workers’ compensation management that create healthier employees and a healthier bottom line for the employers.
Community Health Partners, the Naples Physician Hospital Organization (PHO) in Collier County is comprised of local physicians, hospitals, and other ancillary medical providers who collectively make up a provider network. The physician component of the PHO is also known as Southwest Florida Physicians Association (SWFPA) an Independent Physician Association (IPA). Community Health Partners represents the network in its entirety and establishes contracts on behalf of the network with employer groups, insurance companies, and other networks. Members of these various contracted groups then have access to the best network of physicians and hospitals Collier County has to offer.
The Provider Relations Department will provide assistance and education to the providers who participate in the PHO network and to the members of the community who are seeking medical services from them.
