Our health advocates are here to support you in reaching your health-related goals. When it comes to making healthy changes, Community Health Partners' Health Advocates are the experts. We help you set health goals and learn how to reach them, and also offer support and encouragement to keep you on the road to better health.
Get Started
To begin on your Invest in your Health (change from Healthy Bucks) lifestyle journey, you need to first qualify by scheduling your wellness labs and making an appointment with your Health Advocate. Outside lab results will not be accepted therefore will need to be scheduled through Midland Health by clicking link below. In addition, you can sign up for local classes and wellness appointments by the deadlines in order to qualify.
The qualifying time frame for 2023:
October 1st 2022 - September 30th 2023
Labs and Biometrics must be completed by March 31st, 2023
Coach Meeting or Wellness Consult (if appropriate) must be completed by June 30th, 2023
All additional qualifiers such as educations and preventative screenings must be completed by September 30th, 2023.

What if I Use Tobacco?
Commit to Quit Tobacco Cessation Program
If you stated that you are a tobacco user, or if cotinine (nicotine) was found in your lab profile, you will be asked to participate in the "Commit to Quit" program. You can sign up for the program at the Med Center located on the main campus in the Risk Management building, or the north Med Center at 4513 Executive Drive (off Immokalee Road).
The program offers:

FREE one on one visits with clinician including exam.

FREE one on one visit with certified dietitian.

FREE education materials.
-
FREE cessation products (prescriptions, over-the-counter gums, patches etc..)

FREE clinic appointments - up to one hour without incurring sick leave.
