Community Health Partners administers the Collier County Healthy Kids Program, servicing school age children through this state-sponsored comprehensive health insurance program with income-based premiums.
To be eligilible for the Collier County Healthy Kids Program children ages 5-19 must be enrolled full time in a Collier County school. They can not have private health insurance or Medicaid.
